Location: In client's own home
Hospital Discharge Service "Bridging the Gap Scheme" aims to support patients at home for up to 6 weeks following a stay in hospital or admission to Accident and Emergency.

Age Concern Swansea currently has the following volunteering opportunity:

Hospital Discharge Service "Bridging the Gap Scheme":

  • Provide reassurance and help rebuild confidence
  • Provide emotional support and a "listening ear"
  • Reporting concerns back to the Scheme Managers
  • Provide practical support such as, tidying up, washing dishes, shopping, paying bills, walking the dog, ironing etc
  • Complete your expenses and activity forms and return them to the office monthly
  • A commitment to visit a patient once/twice a week for up to six weeks
  • Attend any necessary training relevant to your role
  • Promote Age Concerns services and activities for older people

Volunteers should be 18+.  CRB checks will be undertaken.  Training provided.
