Age Concern Swansea currently has the following volunteering opportunity: Hospital Discharge Service "Bridging the Gap Scheme": Provide reassurance and help rebuild confidence Provide emotional support and a "listening ear" Reporting concerns back to the Scheme Managers Provide practical support such as, tidying up, washing dishes, shopping, paying bills, walking the dog, ironing etc Complete your expenses and activity forms and return them to the office monthly A commitment to visit a patient once/twice a week for up to six weeks Attend any necessary training relevant to your role Promote Age Concerns services and activities for older people Volunteers should be 18+
